Omara – Terms of Use
Omara is a wellness platform operated by Melius Wellness LLC. We focus on education, eligibility screening, and non-clinical support to help individuals connect with licensed healthcare providers. Omara does not provide medical care, diagnosis, or treatment. All clinical services are delivered by independent providers through third-party clinical platforms.

These Terms of Use (“Terms”) govern your access to and use of the Omara and Soren websites and any related services operated by Melius Wellness LLC (“Melius,” “we,” “us,” or “our”) (collectively, the “Services”).
Important notice: arbitration and class action waiver
These Terms include a binding arbitration provision and class action waiver that affect your legal rights. Please read Section 15 carefully.
1. Our role and clinical services
Melius is not licensed to practice medicine. Providers exercise independent professional medical judgment and are solely responsible for all clinical decisions, diagnoses, treatment recommendations, prescribing determinations, and compliance with applicable medical laws. Nothing in these Terms creates a provider-patient relationship between you and Melius.
Melius is a non-clinical wellness and technology company. We do not provide medical advice, diagnosis, or treatment. Clinical services are provided by independent licensed providers through third-party clinical partners and their platforms (“Clinical Partners”).
If you believe you have a medical emergency, call 911 immediately. The Services are not for emergency care.
2. Eligibility
You must be at least 18 years old and located in the United States to use the Services. By using the Services, you represent that you meet these requirements.
Clinical services are currently offered in all 50 United States and Washington, D.C. However, specific treatments, medications, and services may not be available in every state or jurisdiction. Availability depends on provider licensure, state law, medication type, clinical appropriateness, and regulatory requirements. Providers are licensed in the state where the patient is physically located at the time of consultation. Service availability is subject to change without notice.
3. How the Services work
- We may ask initial eligibility questions approved by a Clinical Partner to help route you to appropriate care.
- If you are eligible, you may be directed to create an account and complete intake within a Clinical Partner portal or systems.
- Payments for clinical services are processed through the Clinical Partner or its service providers, not Melius, even if a payment flow begins on our Sites.
- Labs, prescriptions, pharmacies, and other third-party suppliers may be involved; your relationship with them is governed by their terms and policies.
4. No medical advice
Content on the Services is for informational purposes only and is not medical advice. Do not rely on information from the Services as a substitute for professional medical advice, diagnosis, or treatment.
5. Compounded Medications
Many prescriptions our partners write involve compounded medications, which are custom-made by licensed compounding pharmacies. These are not FDA-approved or evaluated for safety, effectiveness, or quality. They are not the same as, affiliated with, endorsed by, or supplied by brand-name drugs like Ozempic®, Wegovy®, Zepbound®, Mounjaro®, or similar (those are trademarks of Novo Nordisk and Eli Lilly—we have no connection to them).
Photos of medications on the site are just examples—actual packaging, labels, and appearance can vary depending on the pharmacy. Your provider will talk through any risks with you during your consult. We are not responsible for how the medication is made, delivered, or how your body responds.
Prescriptions, when issued, may include commercially available medications or compounded medications when clinically appropriate and permitted under federal and state law.
6. No Guarantees
Paying for membership or services does not mean you’ll get a prescription, medication, or even be eligible. The intake questionnaire collects your history and symptoms so a provider can review it—there are no guarantees of approval, prescription, or results. Everything depends on your provider’s clinical judgment.
7. Hormone Therapy and Metabolic Treatments
Hormone therapy and metabolic treatments offered through Omara and Soren are regulated medical services. They are not cosmetic, lifestyle, anti-aging, or performance-enhancement products.
All prescriptions are issued solely at the discretion of a licensed clinician following medical intake, clinical review, and, where required, laboratory evaluation. Not all individuals are appropriate candidates for hormone or metabolic therapy.
Failure to complete required intake documentation, laboratory testing, or follow-up evaluations may result in delayed, modified, or suspended prescriptions.
7A. Estrogen, Progesterone, and Other Female Hormone Therapies
Estrogen, progesterone, testosterone for women, and related therapies may be prescribed when clinically appropriate based on symptom profile, medical history, and risk assessment.
Hormone therapy may carry potential risks and side effects, which may include, but are not limited to:
- Breast tenderness or swelling
- Bloating or fluid retention
- Headaches
- Mood changes
- Vaginal spotting or irregular bleeding
- Increased risk of blood clots
- Increased risk of stroke in certain populations
- Increased risk of certain cancers depending on timing, duration, and individual health history
- Gallbladder disease
- Changes in blood pressure or lipid levels
The safety and appropriateness of therapy depend on timing, dosage, delivery method, and individual health factors.
If systemic estrogen is prescribed to individuals with a uterus, progesterone is typically required to reduce the risk of endometrial hyperplasia and cancer.
All hormone therapy requires clinician oversight and may require periodic monitoring.
7B. Testosterone Therapy
Testosterone therapy is prescribed only after appropriate laboratory evaluation confirms medical necessity or clinical indication.
Testosterone may be prescribed in various forms, including injectable, topical, oral, or stimulation-based therapies. The specific formulation, dosing, and monitoring schedule are determined solely by the prescribing clinician.
Testosterone therapy:

- May suppress natural testosterone production
- May impact fertility and sperm production
- Requires baseline and ongoing laboratory monitoring
- Is considered long-term, clinician-managed care
Potential risks and side effects may include, but are not limited to:
- Acne or skin changes
- Fluid retention
- Mood changes or irritability
- Elevated red blood cell count (polycythemia)
- Changes in cholesterol levels
- Worsening of sleep apnea
- Gynecomastia
- Cardiovascular risks in certain populations
Patients are responsible for completing required laboratory testing and follow-up appointments. Prescriptions may be delayed, adjusted, or discontinued if monitoring requirements are not met.
7C. Metabolic and Microdosed Therapies (Including GLP-1 / GIP-Based Programs)
Metabolic support programs, including microdosed GLP-1 and/or GIP-based therapies, are prescribed only after clinical review.
These therapies are not intended for rapid weight loss, cosmetic purposes, or short-term aesthetic goals. They are designed for clinician-guided metabolic support when appropriate.
Potential risks and side effects may include, but are not limited to:
- Nausea
- Gastrointestinal discomfort
- Appetite changes
- Fatigue
- Headache
- Injection site reactions (when applicable)
Certain medical histories, including but not limited to pancreatitis, thyroid conditions, or advanced diabetes, may make these therapies inappropriate.
These treatments are not substitutes for comprehensive medical care for diabetes or other chronic conditions unless explicitly directed by a licensed clinician.
7D. Individualized Risk
All hormone and metabolic therapies carry individualized risks. Suitability depends on personal medical history, family history, medications, and overall health status.
Clinical decisions are made solely by licensed medical providers. Participation in care requires honest disclosure of medical history and compliance with monitoring requirements.
8. Accounts and communications
If we offer accounts for non-clinical services, you are responsible for maintaining the confidentiality of your credentials and for all activity under your account. We may communicate with you by email or SMS as described in our Privacy Policy.
9. Payments and subscriptions
Melius may offer optional, non-clinical services through the Services. If applicable, pricing and billing terms for such non-clinical services will be presented at the time of purchase or checkout.
Clinical services, including but not limited to medical consultations, provider evaluations, prescriptions, medications, laboratory services, and subscription-based treatment programs, are provided and billed by independent Clinical Partners and their service providers. Melius does not provide, bill for, or collect payment for clinical services, even if a payment flow is initiated through the Services.
By using the Services, you acknowledge and agree that all charges related to clinical services are governed by the applicable Clinical Partner’s terms, policies, and billing practices.
9A. Subscriptions for Clinical Services
Certain clinical services may be offered on a subscription basis by Clinical Partners. These subscription services may include access to providers, treatment programs, medications (where prescribed), and ongoing support.
Subscription services are initiated when you complete required intake steps and are accepted into a program by a Clinical Partner. Billing may begin at that time and continue on a recurring basis unless cancelled in accordance with the Clinical Partner’s policies.
Melius does not control subscription terms, billing cycles, pricing, or service inclusions for clinical services.
9B. Cancellation of Clinical Subscriptions
You may request cancellation of a clinical subscription at any time; however, cancellation requests are subject to the terms and timing requirements established by the applicable Clinical Partner.
Clinical Partners may require advance notice prior to the next billing date in order to stop a recurring charge. If such notice requirements are not met, you may be charged for the next billing cycle, and cancellation may take effect at the end of that cycle.
Following cancellation, you may continue to have access to clinical services through the remainder of your active billing period, as determined by the Clinical Partner.
All cancellation requests, including timing, processing, and confirmation, are handled by the Clinical Partner and not Melius.
9C. Refund Policy for Clinical Services
Refund eligibility for clinical services is determined solely by the applicable Clinical Partner and is governed by their policies.
In general, and subject to applicable law:
- Fees for clinical services are non-refundable once services have been initiated or rendered.
- If a licensed provider determines that you are not medically eligible for a service after intake or evaluation, a refund may be issued in accordance with the Clinical Partner’s policies.
- Refunds, if approved, are typically limited to the most recent billing period and do not apply to prior billing cycles.
Melius does not issue, process, or guarantee refunds for clinical services.
9D. Medications, Pharmacy Fulfillment, and Returns
Prescription medications, if prescribed, are fulfilled by independent, licensed pharmacies.
Due to federal and state regulations governing prescription medications, medications are generally not eligible for return, reuse, or refund once they have been ordered, dispensed, or shipped.
If you receive a medication that is damaged or incorrect, you must contact the dispensing pharmacy or Clinical Partner directly. Replacement of damaged or incorrect medications is handled by the pharmacy in accordance with its policies.
Melius is not responsible for medication fulfillment, shipping, condition, accuracy, or replacement.
9E. Responsibility for Charges
You are responsible for all charges incurred in connection with your use of clinical services, including subscription fees, provider services, medications, laboratory services, and any related costs.
Questions regarding billing, charges, cancellations, refunds, or medication issues must be directed to the applicable Clinical Partner, pharmacy, or service provider.
10. Prohibited uses
You agree not to:
- Use the Services for unlawful purposes.
- Attempt to gain unauthorized access to systems or accounts.
- Interfere with or disrupt the Services.
- Scrape or harvest data from the Services without permission.
- Use the Services to develop or train AI/ML models using our content or user interfaces without written consent.
- Misrepresent your identity or submit false information.
11. Intellectual property
The Services and all content, trademarks, and materials are owned by Melius or its licensors and are protected by law. You are granted a limited, non-exclusive, revocable license to use the Services for your personal, non-commercial use.
12. Third-party services
The Services may link to or integrate with third-party services (including Clinical Partners, labs, pharmacies, and payment processors). We are not responsible for third-party services, and your use of them is governed by their terms and policies.
If a licensed provider issues a prescription, fulfillment is handled by independent, state-licensed pharmacies. Melius is not a pharmacy and does not compound, dispense, or ship medications. Pharmacy partners are independent third parties responsible for medication preparation, labeling, dispensing, and shipping. Melius does not control pharmacy operations, medication availability, or shipping timelines.

Pharmacy partners may change from time to time based on operational, regulatory, or clinical considerations.
13. Disclaimers
To the fullest extent permitted by law, the Services are provided “as is” and “as available” without warranties of any kind. We do not warrant that the Services will be uninterrupted, error-free, or secure.
14. Limitation of liability
To the fullest extent permitted by law, Melius will not be liable for indirect, incidental, special, consequential, or punitive damages, or for lost profits, even if advised of the possibility of such damages.
To the fullest extent permitted by law, Melius’s total liability for any claim relating to the Services will not exceed the greater of (a) $100 or (b) the amount you paid Melius in the prior 6 months for the Services giving rise to the claim (if any).
15. Indemnification
You agree to indemnify and hold harmless Melius and its affiliates, officers, directors, employees, and agents from claims arising out of your use of the Services or violation of these Terms.
16. Termination
We may suspend or terminate access to the Services at any time for any reason. You may stop using the Services at any time.
17. Governing law
These Terms are governed by the laws of the State of Texas, without regard to conflict of law principles.
These Terms govern your relationship with Melius Wellness LLC only. Disputes regarding clinical care, prescriptions, pharmacy services, laboratory services, or provider conduct are governed by the applicable Clinical Partner, provider, pharmacy, or laboratory agreements and are not governed by these Terms.
18. Arbitration and class action waiver
Informal resolution first.
Before initiating arbitration, you agree to contact us at hello@omarahealth.com and provide a brief description of the dispute and the relief you seek. If we cannot resolve the dispute within 60 days, either party may initiate arbitration.
Binding arbitration.
Except for disputes that qualify for small claims court or requests for injunctive relief relating to intellectual property, any dispute arising out of or relating to these Terms or the Services will be resolved by binding arbitration on an individual basis.
No class actions.
You and Melius agree that disputes will be brought only in an individual capacity and not as a plaintiff or class member in any purported class, collective, or representative proceeding.
Arbitration location and rules.
Arbitration will be conducted in Texas, unless the parties agree otherwise, under rules of an established arbitration provider (to be specified). Judgment on the award may be entered in any court of competent jurisdiction.
Severability.
If any portion of this Section is found unenforceable, the remainder will remain in effect to the maximum extent permitted by law.
19. Changes to these Terms
We may update these Terms from time to time. We will post the updated Terms and update the “Last Updated” date. Your continued use after changes become effective constitutes acceptance.
