What it is. The All-in-One Hormone Cream is a compounded bioidentical hormone therapy that combines estrogen (estradiol/estriol) and progesterone in one cream, with testosterone added when your clinician finds it appropriate. Bioidentical hormones are chemically identical to the ones your body makes. Custom compounded creams are bioidentical but are not FDA-approved.
Who it’s for. Peri- and post-menopausal women with symptoms such as hot flashes, night sweats, poor sleep, vaginal dryness, mood changes or low libido. It isn’t right for everyone. A history of breast cancer or another estrogen-dependent cancer, unexplained vaginal bleeding, a prior blood clot, stroke or heart attack, active liver disease, a known clotting disorder, or pregnancy can rule it out. Your clinician makes the final decision from your full intake.
How it works. Your formula is individualized from your labs and symptoms:
- Estrogen (estradiol/estriol) and progesterone are combined in one cream, with amounts set by your clinician
- Progesterone protects the uterine lining if you still have a uterus, since estrogen alone raises the risk of lining overgrowth
- Testosterone, when indicated, is most often used for low libido after menopause
A pharmacist reviews your lab results and intake to help determine an appropriate customized formulation before your clinician visit.
What’s included:
- Baseline lab panel: a hormone panel plus standard blood work
- Live visit with a licensed clinician to review your results and plan
- Medication delivered to your home, if prescribed
- A follow-up with your clinician
Full panel bloodwork
Required before starting hormone therapy. Included in your HRT price HSA/FSA eligible
Why we require it
Baseline labs let the licensed clinician who reviews your intake confirm hormone therapy is appropriate and safe for you, set a starting dose, and monitor how you’re doing over time.
What the panel checks 11 markers · blood draw at a location in a national lab network
FSH (follicle-stimulating hormone)
Rises as the ovaries wind down, so it helps show where you are in the menopause transition.
LH (luteinizing hormone)
Works alongside FSH and adds context on the transition and on other causes of cycle changes.
Estradiol
Your main estrogen. Gives a starting reference point before estrogen is considered and something to compare against later.
Progesterone
A baseline before progesterone is considered. For women with a uterus who use estrogen, progesterone helps protect the uterine lining.
Total testosterone
Checked before testosterone is ever considered, so any plan can aim to stay within the normal female range, and rechecked over time.
DHEA-S
An adrenal hormone the body uses to make testosterone and estrogen. It adds context to your androgen levels.
TSH (thyroid-stimulating hormone)
Screens thyroid function. Thyroid problems can cause fatigue, mood, weight and temperature changes that look a lot like menopause.
Free T4
The main thyroid hormone in your blood. Helps confirm what TSH suggests.
Free T3
The active form of thyroid hormone. Rounds out the thyroid picture.
TPO antibodies
Can flag an autoimmune thyroid condition such as Hashimoto’s, sometimes before other thyroid levels change.
Prolactin
A pituitary hormone. High levels can cause irregular periods and hot flashes, so checking it helps rule out another cause.
Good to know. If your symptoms are improving, no additional labs are required. Your clinician schedules a follow-up and sets any repeat labs. Timing and fasting instructions come with your lab order. Your clinician decides the final panel. Lab results are information, not a diagnosis. The licensed clinician who reviews your intake decides whether hormone therapy is right for you.
Already on hormone therapy?
If you’re moving from another telehealth service, or you have recent lab results, you start the same way as everyone else, with the quiz. Labs are required before a prescription. The licensed clinician who reviews your intake may accept recent results, at their discretion.
